  • Communication Data: Records of phone calls, emails, and, specifically, data related to the SMS messages you send or receive from us.

 

2. How We Use Your Information

The information we collect is used solely for the following business purposes:

  • Providing and Coordinating Care: Scheduling appointments, developing treatment plans, and communicating with your other healthcare providers (only with your explicit consent).

  • Administrative and Billing: Processing payments, submitting claims to insurance providers, and maintaining accurate patient records.

  • Communication: Sending appointment confirmations, reminders, follow-up instructions, and, if you opt-in, promotional updates or general information about our services.

 

3. Data Security and Sharing

Your privacy is paramount. Elevated Physical Therapy employs reasonable physical, technical, and administrative safeguards to protect your personal and health information from unauthorized access, disclosure, or misuse.

We will not share, sell, rent, or trade your personal or medical information with any third parties for marketing or promotional purposes.

Exceptions to Sharing: Your information will only be disclosed in the following limited circumstances:

  1. Treatment, Payment, and Healthcare Operations: As necessary to provide your care or manage billing, in accordance with our Notice of Privacy Practices.

  2. Legal Obligation: When we are required to do so by law, such as in response to a court order, subpoena, or valid legal process.

  3. To Protect Rights: When necessary to protect the rights, property, or safety of Elevated Physical Therapy, our patients, or others.

  4. With Your Consent: If you provide explicit, written authorization for a specific disclosure.

 

4. SMS Terms & Conditions

These SMS Terms & Conditions govern the text message communications you receive from Elevated Physical Therapy (the "Service").

A. Opt-In and Consent

By providing your mobile phone number and opting in, you agree to receive recurring text messages from Elevated Physical Therapy regarding appointments, scheduling, general updates, and potentially promotional offers. You understand that consent is not a condition of purchase.

B. Message Frequency

Message frequency will vary based on your interaction with our services (e.g., more frequent messages around initial scheduling and less frequent messages during steady-state treatment).

C. Costs and Carriers

Message and data rates may apply. Check with your mobile service provider for details on your specific text messaging plan. Elevated Physical Therapy is not liable for any delays or failures in the receipt of any SMS messages, as delivery is subject to effective transmission from your mobile service provider.

D. How to Stop (Opt-Out)

You can cancel the SMS service at any time. Text STOP to the number you are receiving messages from. After you send the message STOP to us, we will send you one final text message to confirm that you have been unsubscribed. After this, you will no longer receive SMS messages from that program. If you want to join again, just sign up as you did the first time or text START.

E. Help and Support

If you are experiencing issues with the messaging program, you can reply with the keyword HELP for more assistance, or contact us directly using the information provided in Section 5.

F. Privacy

We will use the information you provide through the Service to operate the program and provide services to you. However, we reserve the right at all times to disclose any information as necessary to satisfy any law, regulation, or government request, to avoid liability, or to protect our rights or property.

  • Access your information: Request a copy of your clinical and billing records.

  • Request restrictions: Ask us to limit the way we use or disclose certain information.

  • Request amendments: Ask us to make changes to your records if you believe they are incorrect.

  • Opt-Out of SMS: Use the STOP command as detailed above to stop receiving text messages.
